The Wyoming Skill Based Amusement Games Operator Application is located below. The application can be printed out or completed on your computer, but ALL SIGNATURES MUST BE NOTARIZED BY COMMISSION STAFF. All checks need to be made payable to State of Wyoming Gaming Commission. Please be prepared with all paperwork and payment, so your application can be processed in a timely manner. The Wyoming Gaming Commission is located at:
